- PIAA Physical Form (due Jan 30, 2012)
- Villanova Boathouse Waiver (due Jan 30, 2012)
- Harriton Athletic Training Rules (due Jan 30, 2012)
- Crew Seasonal Information Form* (due Jan 30, 2012)
* Completion of the Crew Seasonal Information Form will facilitate the creation of daily boat line-ups and will serve as the master receipt of the acceptance of policies outlined by the Harriton Crew General Guidelines, Athletic Training Rules, and Villanova Boathouse Standard Operating Procedures. Please return all forms together in the original packet.
Please download and complete the forms as soon as possible. All completed forms should be placed in an envelope with the student’s name, the word CREW prominently displayed, and handed in to the Harriton Athletic Office. Please turn in all completed forms in one packet at the same time.
